The Medical Cannabis Caregiver Application page displays.
To Complete Your Pending Application:
Click the Complete Application button on the Complete Your Pending Application icon on the Caregiver portal.
Enter the required information (with a red asterisk *) in the Caregiver Details section and click the Next button.
Under the Proof of Identity section, you can upload a photo or choose the Photo Affidavit Waiver option. Follow either one of the instructions mentioned below:
Select your Identification Type (Driving License/State ID Card/Passport/Other) from the drop-down menu and click the Upload Files button to add your documentation to the system.
Note: You can download the photo affidavit form from the Click here link.
Enter your Address or click the I don’t have a permanent address checkbox if you don’t have this information, click the Next button.
Note: Applicants must provide their Homeless certification if they select the I don’t have a permanent address checkbox.
Click the Upload Files button to add your supporting documentation to the Proof of Residence page and click the Next button.
Note: The Proof of Residence section displays. If your address differs from your Driver’s License/State ID, upload your proof of residence.
On the Final Application Review page ensure all information was entered correctly and click the Next button.
Notes:
Enter your signature on the Caregiver Consent page and click the Submit button. The Submission Confirmation message displays.
Click the Go to Dashboard button. The Application Dashboard displays.
